The idea for Perfect Health comes from the personal experience of a group of healthcare leaders who struggled to care for loved ones and patients with serious health issues. They confronted a system that was too fractured, too rigid, too disconnected to provide the care they needed. And they did something about it.
First, by bringing together the highest quality and most compassionate care for their loved ones and patients. They reached out to find the best specialists and the most promising traditional and non-traditional treatments. They found caregivers who understood their full range of needs. Their rule was that good wasn’t good enough. The care had to be perfect.
Now, they are bringing that approach to people with on-going, multiple or serious health needs by launching Perfect Health.
How Does It Work?
This group of leaders has built a new model of healthcare from the ground up. A model focused entirely on people with on-going, multiple or serious health needs. Gone is the fee-for-service model of medical billing. Gone is the rigid 15 minute appointment that rushes doctors and patients in and out. Gone are the days where the patient has to try to untangle the confusion between their primary care doctor, specialists, the pharmacy and the insurance company.
At Perfect Health, you’re not just a patient; you’re a partner with your health care team of a primary care physician, nurse, social worker, health coach and any other needed team member. We work with you to identify your goals for your health. Then make a plan together to achieve those goals. We coordinate with all parts of the health system – medical, behavorial, long term care, pharmacy, etc. We help you with claim forms. We follow up between appointments to be sure you’re getting the support you need.
